Individualized Supported Living (ISL)

Individualized Supported Living program provides customized, in-home services 24 hours a day.

This gives consumers with development disabilities the opportunity to live in a shared space arrangement. Typically, 2 or 3 people with developmental disabilities are placed together as roommates sharing a common living area. Emphasis is placed on individuals choosing where they reside with and whom they reside, and the type of community activities in which they wish to be involved. Supports are developed based on individualized needs and focus on many interest areas such as: recreational, social, medical, personal care, transportation or financial/budget planning.


Supports may include:

Menu planning ideas for healthier living

Building cooking skills

Roommate and house searches

Transportation

Help with doctor visits, keeping track of prescription medications

Help with advocacy

Budgeting support and managing bill payment

Problem solving

Social and recreational activities
